The London Centre of Hope Family Health Team is now accepting new patients.
Registration is by APPOINTMENT ONLY on the following dates:

From Monday, February, 1, 2010 to Friday, March 26, 2010
Monday to Thursday 1:00 – 4:00 p.m.
Every Friday 8:30 – 11:00a.m. and 1:00 – 3:30 p.m.
(Times subject to change)

Please phone 519-661-0343 extension 225 to book an appointment to register.

These dates will be for signing up with the London Centre of Hope Family Health Team only and not for seeing the Health Care Provider.
